Only HR and Team HR administrators can create form templates.

Create a form template

To do so, you should reach the Campaigns > Form templates tab on the administrator space.

Then, you should chose what kind of reviews you want to create the form template for:

  • 1-on-1 review: review between an employee and his/her manager (examples: annual review, mid-year...)

  • Private review: review shared by the reviewer to the administrators about an employee (example: inputs for people review)

  • 360 review: collective assessment of an employee by various people (his/her manager, his/her peers, his/her team...)

  • Poll: employees survey.

You can design the questionnaire creating sections, which allows you to organize your form into chapters, and questions that can have different formats such as text,table, linear scales...

On 1-on-1 reviews, it is possible to have the employee's past objectives displayed in the review to analyze them (1) and also to define new objectives (2).

Finally the form template can be saved as private, which means that it will only be seen by the user who created it, or save it as public (by default) which will give access to it to all administrators and managers.

Limits for each type of question

When you are creating a form, each type of questions has a limited number of options / levels / rows / columns. The limits are the following:

  • Linear scale > Maximum of options: 6

  • Dropdown > Maximum of options: 20

  • Checkbox > Maximum of options: 20

  • Table > Maximum of columns: 4 + comments field / Maximum of rows: 12

  • Checkbox table > Maximum of columns: 6 + comments field / Maximum of rows: 12

  • Multiple ratings > Maximum of levels: 10 / Maximum of rows: 24

Move a question

It is possible to move a question in a form template using the arrows that appear when passing over the relevant question.

Still, it is not possible to move a section.

Duplicate a form

From the Campaigns > Form templates tab, it is possible to duplicate a form to use it to create a new form.

Edit a form

From the Campaigns > Form templates tab, it is possible to edit a form through the pencil icon that appears when passing over.

Make a private questionnaire visible to another user

It is possible to give visibility to another user on one of your private form templates from the tab Campaigns > Form templates

You will have to write the names of the people with whom you want to share the private templates.
